Remote Monitoring Plans

Remote Monitoring for Diabetes and Thyroid Patients: We offer various programmes for remote monitoring of diabetes and thyroid patients. All you need to do is:

  • Subscribe to one or more of our diabetes or thyroid remote monitoring programmes.
  • Log in when required, to enter your blood sugar/insulin or thyroid values as instructed.
  • The values will be reviewed and you will receive feedback online.

Through our remote monitoring programme, we can help you stay on track as you follow our treatment plan, thus improving your diabetes/thyroid control over a sustained period. Payments for the programmes can be made online. Call us at +91 9847148000 if you have any questions.

Basic Diabetes
This programme is meant for patients with diabetes on Oral Anti Diabetic (OAD) medicines. In this programme, the patient can report blood sugar, insulin doses, dietary changes, blood pressure, lipid profile and weight readings.
Monitoring health trackers

On subscribing to this package, you will be required to report the test readings for the following trackers.

Trackers Reporting by Patient** Frequency of Monitoring and Review
Blood Pressure Once a month ( if controlled) Only one response per month
Blood Sugar Fasting and post-meal blood sugars to be reported at least twice a week Only one response per month
Food Once a month or if any change responsible for low or high sugars Only one response per month
Cholesterol Once in 3 months Once in 3 months
Weight Once a month Only one response per month
** General Instructions to Patients
  1. For optimal dose titrations, the patient should enter at least 2 blood sugar profiles weekly.
  2. DO NOT forget to mention to the monitoring doctors if you have any hypoglycemia conditions
  3. Calibrate or troubleshoot your self-monitoring device before conducting any medical tests.
  4. In case of acute or emergency conditions, DO NOT rely on the Tracker readings or feedback. Please contact your nearest hospital immediately.

This programme is meant for patients with diabetes on oral anti diabetic medicines with thyroid disease. In this programme, the patient can report blood sugars, insulin doses, dietary changes, blood pressure, lipid profile and weight readings.

Intensive Diabetes
This programme is meant for patients with type 1 or type 2 diabetes on insulin pump or for those requiring intensive monitoring (for example: patients with post CABG or ongoing infection). In this programme, the patient can report blood sugars, insulin doses, dietary changes, blood pressure, lipid profile and weight readings.

On subscribing to this package, you will be required to report the test readings for the following trackers.

Trackers Reporting by Patient** Frequency of Monitoring and Review
Cholesterol Once in 3 months Once in 3 months
Food Once a month or if any change responsible for low or high sugars One response per week.
Blood Sugar Fasting, pre meal and post meal + 3 AM values to be reported. One response per month.
Weight Once a month One response per month.
Blood Pressure Once a month ( if controlled) One response per month.
** General Instructions to Patients
  1. For optimal dose titrations, the patient should enter at least 2 blood sugar profiles weekly.
  2. For a patient undergoing the insulin pump treatment, additional values expected to be recorded will include pre meal and 3 AM values ( i.e. fasting, pre lunch, pre dinner, post breakfast, post lunch, post dinner and 3 AM values).
  3. Patients are requested to enter the carbohydrate content of each meal to help adjust insulin doses better. The more the number of entries, the better it will be for the monitoring doctors to analyse the data.
  4. Calibrate or troubleshoot your self-monitoring device before conducting any medical tests.
  5. In case of acute or emergency conditions, DO NOT rely on the Tracker readings or feedback. Please contact your nearest hospital immediately.

This programme is meant for patients having hypothyroidism. With this programme, you can report the results of your thyroid function tests (T3, Free T4 and TSH) every 3 months.

Trackers Reporting by Patient** Frequency of Monitoring and Review
Thyroid One thyroid test will be reported every 3 months. In pregnant patient, it may be 2 months. Only one response per 3 months
** General Instructions to Patients
  1. In case the patient is pregnant, the frequency of monitoring by doctors will be done every 6- 8 weeks. The programme will be modified accordingly.
  2. Calibrate or troubleshoot your self-monitoring device before conducting any medical tests.
  3. In case of acute or emergency conditions, DO NOT rely on the Tracker readings or feedback. Please contact your nearest hospital immediately.
